Turn our PDF enrolment forms into guided forms

Our enrolment packet is a PDF. Students print it, fill it in by hand, scan it and email it back, and then someone retypes everything. Lots of answers come back with "what does this mean?" written in the margin.

We'd love to start from the PDF, get an online form with help on every question, and have the answers filled back into our original document, so the office still gets the PDF they're used to.
